Annual Forney ISD Physicals Scheduled for May 12 at City Bank Stadium
All Forney ISD secondary level students involved in athletics, band, cheer and drill will have to have a new physical for the 2018-19 school year. Forney ISD will conduct physicals for students on Saturday, May 12 at City Bank Stadium. Students that plan on participating in athletics, band, cheer or drill from Brown Middle School, Warren Middle School, Forney High School and North Forney High School should attend the physicals.
Physical exams will be held for current 6th, 7th, and 8th grade students from Brown Middle School will be from 8 a.m. to 9:30 a.m. and North Forney High School physicals will be held from 9:30 a.m. - 11 a.m. Warren Middle School physicals will be from 12 noon to 1:30 p.m. and Forney High School physicals will be held from 1:30 p.m. - 3 p.m.

Brown Middle School - 8 a.m. - 9:30 a.m.
North Forney High School - 9:30 a.m. - 11 a.m.
Warren Middle School - 12 noon to 1:30 p.m.
Forney High - 1:30 p.m. - 3 p.m.
Students should bring the medical history and physical form with them to the physicals. The forms can be accessed on the Forney High School Athletic website at jackrabbitathletics.com and on the North Forney Athletic Website at gofalconathletics.com. Below are links to both athletic websites.
The medical history form must be completed in advance for the doctors to review.
There will be a $10 cash fee upon check-in for the physical evaluation.
